If your child is determined eligible based on assessment and evaluation, you and the early intervention professionals will work as team to develop a plan called the Individualized Family Service Plan (IFSP).

As a parent, you will help the team develop goals and outcomes that will promote your child’s learning and development.  Services are provided in the home and community by a Team Lead.
